You're like me! i have to look out for me when i get home and make sure i have enough, but when I'm there i just get dilaudid in the IV first so it will work fast then get a shot so it will last long.
I AGREE WITH SOME OF THE OTHERS I GO TO THE ER THEY START OUT WITH DILAUDID AND IV FLUIDS I USEALLY GET ABOUT 2OR 3 ROUNDS OF THAT AND BY THAT TIME I'M SO OUT OF IT THAT IF I'M STILL IN PAIN THE ADMITE ME BUT WHEN I DO FEEL WELL ENOUGH TO GO HOME I SLEEP FORR ABOUT 2OR 3 DAYS
IV fluids, a Dilaudid PCA, usually set at 1-2.5 mg on an 8 minute lockout (u can press the button to inject meds in the IV every 8 mins). They also prescribe anti-nausea meds as needed, and I usually ask for an NSAID like Toradol if my joints are inflammed along with my normal pain that presents in a crisis. I have a high tolerance and take other meds for chronic pain, so a little shot here and there q 4 hours don't do NOTHING. Plus, at the hospital I am seen at that is their protocol-to use PCA's not having a nurse come in and give injections as needed every 3-4 hours.
I hate the prn system anyway because many nurses will catch an attitude if you are calling them around the clock for pain medication. That was typically the case for me when I would receive prn boluses back in the day, because the pain was not being properly managed. Thank God those days are over.
After about 15 mgs of Dilaudid/4 hours, if I am not feeling better, the docs usually admit me. Haven't had an admission since Nov. 2008, no ER visits since Feb. 09. Thank the Lord.
IV fluids, morphine, and totordol or however u spell it, its like motrin except more powerful, i rarely get blood transfusions and i dont really like taking pain medication unless i reall REALLY need it, im trying to jus deal with it so i wont be jus stuck on pain meds tha rest of my life cuz one day i might not be able to get them, and then ill be messed up, plus that stuff make me constipated.....
for me its PCA Dilaudid about 2-3 mg every 10 min
oral methadone
IV toradol
aprox.2 lit of O2
iv fluids (full blast)
iv benadryl
and something PRN for nausea
Rocephin for infection if needed
blood if needed
and a few hot nurses lol
i get dilaudid for pain they used to do it through PCA now its prn by the nurses every 3hrs, fluids, something for itching & nausea and blood if needed which in most cases it is! thats all folks it hasn't changed in years except back when I was little it was demerol....

sorry for such a late response, but i joined the committee recently. when your wife have crisis start with immediate IV fluids, instead of giving morphine directly start with some lower analgesics and if the crisis do not cure than shift to opiates, along with this check for any infection and accordingly give proper antibiotics, soda bicarb is also very useful during crisis.
